Kinstead Health Collective may offer membership-based or program-based care designed to provide ongoing support and guidance.

Nature of Membership

Memberships provide access to certain services such as follow-up visits, messaging, and care coordination.

Memberships are not insurance and do not replace comprehensive primary care or emergency medical services.

Services Included

Each membership or program will clearly outline what is included, which may consist of:

  • Scheduled visits

  • Follow-up support

  • Messaging (with defined limits)

  • Care plan adjustments

  • Wellness and lifestyle guidance

Services Not Included

Memberships do not include:

  • Emergency care

  • Hospital services

  • Specialist care

  • Laboratory fees

  • Medication costs

Medication Policy

Medications, including weight loss medications or GLP-1 therapies, are prescribed only when clinically appropriate and are not guaranteed.

Medication costs are separate and billed through the pharmacy.

Billing & Payments

Memberships or programs may be billed monthly or as a one-time fee.

By enrolling, you authorize recurring charges if applicable.

Cancellation Policy

Memberships are designed to provide ongoing, consistent care and support.

To cancel your membership, a 30-day written notice is required. Your membership will remain active, and you will continue to have access to services during this 30-day period.

Any scheduled payments within this notice period will be processed as planned.

This policy allows for continuity of care and appropriate transition of services.

Membership fees are non-refundable once billed, including during the cancellation period.

Patient Responsibility

Patients are responsible for:

  • Attending scheduled visits

  • Following agreed-upon care plans

  • Communicating changes in health status

Provider Discretion

All medical care, including treatment plans and prescriptions, is at the discretion of the provider.

Kinstead Health Collective may, at its discretion, allow exceptions in certain circumstances.

Agreement

By enrolling in a membership or program, you agree to the terms outlined above.
